We are seeking a dedicated and compassionate Regional Registered Nurse Care Coordinator to join our healthcare team. This is a regional position covering both The Ivy at Davenport in Davenport, Iowa and The Silver Oak Nursing and Rehabilitation in Marion, Iowa.
The ideal candidate will play a vital role in ensuring that residents receive high-quality care throughout their treatment journey. This position involves coordinating resident services, managing referrals, and being a key communication between resident and healthcare providers. The RN Care Coordinator will work closely with nursing staff and other healthcare professionals in both the pre-admission process, during the admission process and throughout the stay to enhance resident experience and outcomes.
Duties
- Coordinate patient care activities, ensuring seamless transitions between different levels of care.
- Serve as the primary point of contact for residents, addressing inquiries and providing information regarding their care plans.
- Maintain accurate medical records, ensuring that all resident information is up-to-date and compliant with healthcare regulations for admissions.
- Collaborate with nurses, physicians, and other healthcare professionals to develop and implement individualized care plans based on resident needs.
- Monitor patient progress and follow up on treatment plans to ensure adherence to prescribed therapies.
- Educate residents about their health conditions, treatment options, and available resources to empower them in their care journey.
- Utilize knowledge of physiology to assess resident needs effectively and provide appropriate support.
Experience
- Previous experience in Admissions and Care coordination service roles is preferred.
- Familiarity with medical records management is essential.
- Background in Admissions, Clinical Leadership and Skilled Nursing Facility is preferred.
- Knowledge of physiology is a plus, enhancing the ability to understand resident conditions and coordinate appropriate care.
- Strong communication skills are necessary for effective interaction with residents, families, and healthcare teams.
- Ability to work collaboratively in a fast-paced environment while demonstrating empathy and professionalism.
