Key Responsibilities:
- Develop and implement comprehensive social services programs to meet residents’ emotional, social, and psychosocial needs.
- Conduct assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
- Serve as an advocate for residents and families, ensuring clear communication and access to community resources.
- Maintain compliance with state and federal regulations related to resident care.
- Train and supervise social services staff, fostering professional growth and teamwork.
- Address resident and family concerns with empathy and effective problem-solving.
Qualifications:
- Bachelor's degree in Social Work (BSW) or a related field; Master’s degree preferred.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Pennsylvania (or eligibility for licensure).
- Minimum of 1-2 years of experience in a healthcare or long-term care setting; supervisory experience is a plus.
- Knowledge of state and federal regulations governing skilled nursing facilities.
- Strong communication, organizational, and interpersonal skills.
