Director of Case Management - Acute Care
the Nursing Director of Case Management, reports to the VP of Clinical case Management and has 35 Staff .
A full-service medical and surgical acute care center. We offer comprehensive cardiovascular care, oncology, orthopedics, and neurosciences. We offer minimally-invasive laparoscopic and robotic surgery, weight loss surgery and treatment. Other services that we provide include women’s health and wound therapy. We are fully accredited by the Joint Commission and certified as a Comprehensive Stroke Center. Our hospital has an Accredited Chest Pain Center by ACC (American College of Cardiology) and is designated as a Blue Distinction Center for Spine Surgery and Bariatric Surgery. The Case Management Director is accountable for the overall coordination of assessment, planning development, implementation and evaluation of the clinical operations within Case Management Department.
The role is responsible for patient throughput, care coordination, post discharge, case management team, social work, length of stay, revenue cycle and bed management.
Directs department functions in accordance with state, federal, local regulatory requirements as well as policies and procedures of the Health Center.
Regularly meets with Physician Leaders and CNO to review progress of strategic goals, operational performance metrics, improvement strategies and unit specific concerns.
Ensures departmental metrics and goals are maintained and outcomes are measured and support organizational and nursing mission, vision, values and goals.
Promotes empowerment of the departmental managerial staff by utilizing collaborative approaches and encourages new ideas.
Functions as a leader of the healthcare team and various interdisciplinary committees to improve patient care and meet patient needs.
Hires educates, mentors, and evaluates responsible department managers. Educates managers in new and existing departmental procedures and health systems.
Identifies and implements measure to achieve evidence based outcomes and organizational goals for areas of responsibility.
Qualifications :
Registered Nurse in the State of FL
MSN or MBA preferred
Certified Case Manager Certification -CCM preferred
5 years experience in an clinical nurse acute care setting within a multiple interdisciplinary environment.
2 years Supervisory, management experience in Case Manager, Care Coordination role.
Advanced Utilization Review process
Experienced with staff development, workflow and strategic planning
Patient care coordination
Minimum 2 years of Director level in Case Management
Bed Management and Patient throughput

Patient length of stay
Score cards, quality metrics experience
Strong hospital software application and Electronic Medical Records experience.
