What are the responsibilities and job description for the Patient Outcomes Coordinator, RN position at Jackson County Regional Health Center?
Patient Outcomes Coordinator: Responsible for assisting, coordinating and communicating information necessary to third party Payers including securing authorization for clinical care and providing accurate, timely discharge notification to HIM and external agencies. Completes the cycle of precertification, compiles medical necessity documentation and communication of clinical authorizations. Retrospectively ensures the patient's progress in the acute episode of care through post discharge and is quality driven while being efficient, cost effective, and providing a safe discharge plan. Works with physicians to facilitate effective and efficient transitions through the process of hospitalization. Works collaboratively with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by Payers, and that resources are efficiently and effectively utilized. Serves as a liaison between the hospital and community agencies of facilities for the exchange of clinical and referral information. Works closely with patients and family members throughout their hospitalization. Assist with care coordination needs for all hospital patients beyond the Patient Care Unit when requested. Continue to work on and grow the Post-Acute Stroke Rehab Program with collaboration of the American Heart Association.
Position is a 1.0 FTE with a M-F work schedule, with occasional off shift requirements as needed for position.
Qualifications Include: Baccalaureate in Nursing, BLS, ACLS, PALS, Dependent Adult & Child Mandatory Reporter. Word processing, Cerner, interpersonal skills, excellent customer service skills required.
Requirements: This position requires a minimum of 3 years' experience as a staff RN in an acute setting. Utilization Review experience preferred.
