What are the responsibilities and job description for the SOCIAL WORK - Department on Aging position at Jackson County?
Provide services for older adults including intake and comprehensive assessments of clients in the home environment to obtain information concerning health, financial resources, and community and family support systems. Responsible for case coordination, development of care plans and referrals to, or assistance with, other service agencies. SUMMARY OF RESPONSIBILITIES:
1. Performs outreach and case finding of the target population as defined by the grant(s).
2. Determines eligibility for services using a complex set of criteria, and makes recommendations and/or referrals for interventions.
3. Develops a service plan based on input from the client, health care providers, members of the client’s family, and members of the client’s social network.
4. Monitors implementation of service plans for each assigned client, providing regular follow-up and assessment of services in relation to goals and objectives of the service plan.
5. Seeks potential resources for clients who have needs beyond services that are normally available and/or are available on a limited basis.
6. Coordinates support services as assigned.
7. Makes referrals to services such as housing, legal advice, medical/ psychological care, and financial assistance. Works with agencies to obtain services for clients such as Social Security, Department of Health and Human Services, Veterans Administration, and so forth.
8. May also: coordinate and facilitate support groups and work individually with participants; receive training to provide services through the State Health Insurance Program (SHIP) and Medicare Improvements for Patients and Providers Act (MIPPA); serve as liaison with community agencies for coordination of services.
9. Maintains complete and detailed case records consistent with Department on Aging standards and requirements of funding sources.
10. Advocates for clients by identifying unmet needs to community and service agencies.
11. Enhances community understanding of department programs through presentations to community organizations and other interested groups.
12. Mentors new team members who are onboarding and learning the assessment process for grant services.
SALARY COMMENSURATE WITH EXPERIENCE:
Education: Bachelor’s degree in a social science or a related field.
Experience: One year of professional experience in a human services agency, preferably providing services for older adults. Working knowledge of grant program oversight preferred.
Other Requirements: Possession of a valid Michigan Driver’s License.
Relevant Skills/Specialized Equipment Knowledge:
- Basic office equipment such as telephone, calculator, photocopier, computer laptop, fax, etc.
- Computer programs including word processing, spreadsheets, and database entry and maintenance.
Schedule is Monday through Friday, 8 am to 5 pm.

The Employee contribution for the RHS Plan is $900 annually with an employer contribution of $2,050 annually.
