Category: Professional City: Jackson State: Tennessee Shift: 0 - Day (United States of America) Job Description Summary: The Social Worker collaborates with all disciplines for the purpose of assessing, planning, implementing, and evaluating/documenting psychosocial factors which contribute to patients’ illnesses and/or have an impact on continuity of care; scope of services may include, but is not limited to, discharge planning, counseling, crisis intervention, information and referral, domestic violence, drug and alcohol abuse, suspected abuse, psychiatric services, and guardianships per regulatory requirements. Services may be provided for inpatient and related outpatient services. Employee is subject to call back and overtime as required by the hospital. ESSENTIAL JOB FUNCTIONS: Completes and documents psychosocial assessments, chemical dependence consults, intervention and coordination of care to identified populations Provides crisis intervention/counseling to identified populations to enhance patient family coping mechanisms regarding illness and adaptation for families to their illness Engages the patient, family/significant other, and interdisciplinary team in the discharge process to identify appropriate post-acute resources and community service follow-ups Provides education to patients/family/significant other and interdisciplinary team in regard to psychosocial factors related to illness and ongoing support of needs Actively participates in those areas of expertise to further enhance the role of social work within the hospital Completes duties as required for the purpose of discharge planning, i.e. options forms, agency referrals, etc. Complies with standards and requirements in medical/legal reporting situations Complies with social work competencies by department and hospital standards Communicates telephonically and electronically with the outpatient providers in an effort to enhance the continuum of care Assume responsibility for WTH required continued education and own professional growth Performs other duties as assigned or required JOB SPECIFICATIONS: EDUCATION: Bachelors of Social Work or equivalent required. MSW preferred. LICENSURE, REGISTRATION, CERTIFICATION: LBSW, LCSW, MSW preferred. Bachelor’s degree in Social Work required.
