Practice / Organization Details :
- Provide exceptional care to patients in a hybrid in-home and virtual model
- Manage a low-volume patient panel to ensure a focus on quality of care
- Established clinical care team including 2 experienced physicians, dedicated APPs, pharmacist, nursing, and social worker support
- Flexible schedule to work around your needs throughout the day - supreme work / life balance offered
- Strong salary range of up to the 70th percentile of MGMA - Anywhere between $120-$150K
