What are the responsibilities and job description for the Nurse Practitioner / PA - Acute Rehab position at Jacksonville Hospitalists?
Duties:
- Provide high-quality, comprehensive primary care services to patients.
- Perform physical examinations, order and interpret diagnostic tests, diagnose and treat common acute illnesses and injuries.
- Develop and implement patient care plans, including discharge planning and coordination of care with other healthcare providers.
- Prescribe medications as needed.
- Collaborate with other healthcare professionals to provide coordinated and integrated care.
- Utilize electronic health record systems to document patient encounters and maintain accurate medical records.
- Stay up-to-date with current medical knowledge and best practices through continuing education and professional development.
Requirements:
- Valid Nurse Practitioner license in the state of Florida.
- Master's degree in Nursing from an accredited program.
- Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
- Experience in providing primary care services across the lifespan, including family planning in an inpatient acute rehab setting / skilled nursing.
- Strong clinical skills and ability to work independently as well as collaboratively within a healthcare team.
- Excellent communication and interpersonal skills.
