Jacksonville Orthopaedic Institute is looking for a Part-time Physical Therapist Assistant / PTA or Physical Therapist / PT to join the Baptist Nassau Rehab team.
PTA : The Physical Therapist Assistant provides physical therapy services to patients rereferred to the center under the supervision of the physical therapist.
PT : Provision of physical therapy services to patients referred to the center. Provide an evaluation and assessment, design a written plan of care, implementation and supervision of treatment programs and educational services.

Essential Duties and Responsibilities
- Provide physical therapy services to patients as directed by the patient care plan and supervised by the physical therapist.
- Ensure completion of all required documentation of patient care activities.
- Adhere to the established policies and procedures of the CENTER.
- Fulfill all duties assigned by either the physical therapist or the Site Coordinator.
- Report to the Site Coordinator any injuries or adverse reactions during or after treatment.
- Participate in patient review conferences and staffing.
- Assist the Site Coordinator as follows, but not limited to : Utilization Review, Quality Assurance, Program Evaluation, Infection Control, and Safety Committee activities.
- May directly supervise assigned tasks of volunteers and PTA students. It is understood that the PTA makes no judgments or changes in student's role in CENTER. This responsibility is approved by the Administrator and Site Coordinator.
- Participate in CENTER in-service activities.
