About the Position:
As the MDS Assistant, you will play a crucial role in the assessment process of our residents' care by collaborating with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident. Your expertise in nursing will help ensure that the highest standards of care are delivered consistently
Duties and Responsibilities:
- Assist in the completion of MDS assessments and ensure accuracy and compliance with regulations.
- Collaborate with the nursing team to develop and update care plans based on resident needs.
- Participate in interdisciplinary team meetings to discuss resident progress and care strategies.
- Monitor resident health status and report any changes to the nursing staff.
- Educate residents and their families about care plans, treatment options, and facility services.
- Maintain accurate and confidential records of resident assessments and care documentation.
- Stay updated on federal and state regulations related to MDS and resident care to ensure compliance.
Requirements and Qualifications:
- Valid and active LVN license in the state of practice.
- Experience in long-term care facilities preferred but not required.
- Strong understanding of the MDS process and its impact on resident care.
- Ability to work effectively in a team-oriented environment.
- Compassionate and dedicated to providing exceptional patient care.
