Responsibilities
- Conduct comprehensive assessments of clients in their home
- Collaborate with agency staff to coordinate care for clients transitioning between different levels of service.
- Maintain accurate documentation of client assessments, treatment plans, and progress notes in compliance with coding standards.
- Engage with children and families, providing support and resources tailored to their unique circumstances.
Skills
- Experience with client engagement.
- Familiarity with outpatient care processes.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
