JAMAICA HOSPITAL MEDICAL CENTER, a major health care facility located in Queens, N.Y. is seeking an Ambulatory Care Social Worker for the Population Health/Ambulatory Care department.

Job Responsibilities:
- The Ambulatory Care Social Worker is responsible for providing comprehensive social work services to patients and their families in an outpatient setting.
- This role involves assessing patient needs, developing care plans, coordinating services, and providing
- support to help patients manage their health conditions and navigate the healthcare system. The social worker will collaborate with a multidisciplinary team to ensure the delivery of high-quality, patient-centered care.
- Provides Care Management services to patients with history of behavioral health diagnoses or concerns and develops interventions to address high or inappropriate utilization
- Completes comprehensive bio-psychosocial assessment, including comprehensive chart reviews for at risk patients, in their home or by phone
- Refers for mental health, and/or substance use disorder services as needed; collaborates with medical and behavioral health providers
Required Skills/Experience:
- Care management and patient advocacy skills
- Familiarity traveling around the community
- Knowledgeable and investigative about financial entitlements, social and community resources
- Excellent communication skills, both written and verbal
- Experience working with diverse populations and addressing complex psychosocial issues.
- Skills: Strong assessment, counseling, and crisis intervention skills; Excellent communication and interpersonal skills;
- Ability to work independently and as part of a multidisciplinary team;
- Proficiency in electronic health records (EHR) and case management software.
Qualifications:
- Minimum of 2-3 years of experience in a healthcare setting, preferably in ambulatory care or outpatient services.
- Master’s Degree in Social Work
- Must Have New York State Licensed Clinical Social Worker, LCSW-C, LCSW, LMSW (LCSW-C preferred)
- Prior hospital or clinic experience a plus

Pay: $38.23 - $51.69 per hour
