ONCOLOGY CLINICAL PHARMACY SPECIALIST

Jamaica Hospital Medical Center, a renowned community-based hospital and Level 1 Trauma Center in Queens, NY, is seeking an Oncology Clinical Pharmacy Specialist for a full-time, day-shift position.
Key Responsibilities:
- Provide clinical pharmacy services for inpatient and outpatient hematology/oncology medication management.
- Manage adult patients receiving ambulatory chemotherapy and oncology-related admissions.
- Collaborate with an interdisciplinary team to enhance patient care and satisfaction.
- Evaluate drug therapy for appropriate use, effectiveness, and adverse drug events (ADEs), intervening as necessary.
- Monitor pharmacokinetics for targeted drugs and assist in ADE prevention and reporting.
- Ensure timely medication access, assist with insurance authorizations, and connect patients to financial assistance programs.
- Educate Oncology team members, residents, and students on oncology pharmacotherapy.
- Serve as a liaison between pharmacy, nursing, and medical staff to improve drug-related policies and guidelines.
- Develop chemotherapy-related treatment plans, order sets, and drug monographs for safe and cost-effective use.
- Support Joint Commission survey preparation and participate in hospital committees.
- Contribute to investigational drug services for oncology research studies.
