Some of our recent projects including the modernization of our Emergency Department, and our affiliation with Memorial Sloan Kettering Cancer Center.
We are currently seeking Psychotherapists for our Center of Excellence Grant ( COE) program in the Mental Health Clinic. The COE grant funded program is for the increase of patients who are high risk for all causes including ED and hospital admissions and readmissions for psychiatric, physical health, and conditions that are exacerbated by Health- Related Social Needs ( HRSNs). The program was funded to increase the intensity, frequency, and duration of medication therapy, psychotherapy, and care coordination services for patients with ( HRSNs).

The Psychotherapists responsibilities include care coordination and management, initial and annual diagnostic assessment, treatment planning, individual, family and group psychotherapy, and discharge planning. They will work closely with COE grant funded Psychiatric Nurse Practitioner to coordinate care for patients with psychiatric, physical health, and HRSNs challenged to prevent all cause ER and hospital admissions and re-admissions.
