You will be responsible for the hospice case management of your assigned patients intermittently in their place of residence. Brief Overview of Responsibilities for Registered Nurses:
Direct Care:
- Perform comprehensive patient assessments and develop individualized care plans with the IDG.
- Provide skilled nursing care and treatments as prescribed.
- Educate patients/families on end-of-life care, healthcare decisions, and dietary counseling.
- Conduct death visits and provide bereavement support as needed.
- Administer medications and stay updated on drug therapies.
Case Management:
- Coordinate all aspects of patient care, including services provided by IDG and contracted providers.
- Ensure continuity of care by managing patient schedules and collaborating with team members to address emerging needs.
- Keep patient care plans up-to-date and ensure documentation is accurate, complete, and timely
- Supervise and evaluate care provided by Hospice Aides and Licensed Practical Nurses (LPNs), conducting supervisory visits as required
Qualifications of Registered Nurses:
- Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of hospice experience is required.
- Graduate of an accredited nursing school.
- Must have a valid driver's license and a reliable car available for use during work hours.
