Brief Overview of Responsibilities for Home Health Clinical Manager:
- Supervise field clinicians and all relevant activities pertaining to episodic management and clinical outcomes and provide progressive discipline as required
- Receive case referrals and assess patients' needs to assign appropriate clinicians
- Ensures accuracy with staff to comply with company policy and procedures
- Provide staff development opportunities to include in-service and educational classes
- Review and investigate the clinical content, diagnoses, medical history, medications, fee status, and other procedures from the records of clients.
- Appraise staff performance during their probationary period and annually with the Director of Nursing/Administrator, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
- Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
- Review each nurse's schedule for recertification, discharges, and attendant supervisory visits.
- May be requested by the Director of Nursing/Administrator to fill in for field staff when necessary.
- Manage staff by reviewing weekly reports of productivity, length of stay, hospitalizations, outcomes, and timeliness of care.
- Other duties as assigned.
Qualifications:
- Preferably two years of home health care experience
- At least 1 year of management/leadership experience, preferably in home health
- Current Virginia State RN License
