James River Home Health is actively seeking a Full-time Intake Coordinator to join our growing team! You will be responsible for managing the intake process for all referrals to James River Home Health from the time the referral is received until the Start of Care visit is completed by clinical staff. This position works closely with the JRHHC marketing, clinical, and medical records staff to ensure that all new patients receive prompt care supported by accurate information about the patients and their care needs.

Full-time Monday-Friday with an on-call weekend rotation

A brief overview of Responsibilities:
- Performs intake for company referrals and Resumption of Care admissions, coordinating the admissions and re-admission process, care coordination with DME, IV Therapy, and other specialized services.
- Answers phone calls related to referrals to JRHHC receives both paper and e-referrals, and enters data into Home Care Homebase to begin the workflow process.
- Conducts a welcome call with each new patient referred to HRHHC; verifies information in the referral form(s) to ensure the accuracy of patient demographic information and insurance coverage.
- Submits reports of all cases not started for a given week at the end of the week to the Leadership team
- Determines patient’s eligibility for services from JRHHC based on the location of the patient’s home and insurance coverage acceptable to the Company.
- Completes daily follow-up on all pending referrals to complete admission/re-admission status
- Other duties as assigned
Requirements:
- High school diploma or equivalent required
- Previous experience in a home health care setting is preferred
- Proficient in basic computer skills with the ability to maintain accurate records
- Strong verbal and written communication skills
