The Scheduler is responsible for coordinating Home Health services for all JRHH patients. You will be responsible for preparing schedules according to patient needs, staff, and care plan.

A brief overview of Responsibilities:
- Develops schedules for the designated clinical team to which this position is assigned
- Ensures the patient schedule follows the frequency as ordered by the physician.
- Schedules visit to appropriate clinicians for all disciplines daily.
- Assign SOCs, ROCs, re-certifications, and discharges to appropriate case managers.
- Assist in ensuring the case management process is followed
- Works closely with clinical staff to organize a manageable schedule in a timely manner
- Monitors productivity in coordinating with Clinical Managers
- Provides the following support for B or Unverified Visits
- Clear visits from the schedule when requested to do so by the Billing Specialist
- Determine why the visit was not completed or verified
- Alert the clinical staff and appropriate Clinical Manager regarding the visit status
- Follow-up with Billing Specialist with results when tasks are completed
- Assist management with documenting changes to the plan of care. (e.g. missed visits) and minimizing changes to those plans
- Performance of other duties as required
Qualifications:
- A minimum of 1 year of home health or hospice scheduling experience is required
- Homecare Homebase experience is a plus!
- Possess a high level of professionalism and integrity.
- Must be highly organized, detail-oriented, and have exceptional planning and problem-solving skills
