Job Title: Licensed Practical Nurse (LPN)
Reports To: RN Supervisor
FLSA Status: Non-Exempt
Division: Community Living Services
Location: Various locations throughout Rockland
Date: March 11, 2024
Purpose: The Licensed Practical Nurse (LPN)-Charge will align his/her scope of work with Jawonio’s Mission, while incorporating Jawonio’s Core Values as the guiding principle for all work activities.
Summary: The Licensed Practical Nurse (LPN)-Charge is responsible for monitoring the health of individuals/participants in the residencies. He/she will perform routine nursing care activities that support quality of care in accordance with the nursing care plan. Provides mentoring and additional support to the Direct Service Professionals as it relates to the health, safety and wellbeing of participants residing at the Residence.
Essential Duties and Responsibilities include the following:

- Performs routine and skilled nursing treatments, first aid, dressings, medication, and enteral feedings & assists direct care with the administration of medications and treatments when needed.
- Assists with the preparation, implementation, and continuing evaluation of the Plan of Nursing Services (PONS) as part of the interdisciplinary team. Serves as a mentor to direct care staff, professionalizing their skills and increasing their knowledge base as it pertains to the PONS and any health-related topic.
- In the absence of house supervisory personnel, assumes a leadership role, directing staff and ensuring the safety and well-being of the Individuals and overseeing the operations of the house.
- Observes, records and reports progress or any changes in the Individuals condition. Documents in nursing reports and medical files all pertinent information. Reports any changes or observations to the RN Supervisor Residence Manager, Family members, Health Care Provider(s), or other professionals as appropriate.
- In the absence of the Manager or Coordinator will assign staff to transport Individuals on routine medical appointments. Will accompany Individuals to medical and psychiatric appointments when warranted.
- Maintains inventory & orders medical supplies and medication on a weekly basis.
- Assists RN Supervisor with training direct care staff on health-related procedures & Plan of Nursing Services (PONS).
- Monitors medical event tracking completed by direct support professionals to ensure compliance and optimal health of participants. Reports any concerns or areas of noncompliance to Residence Manager & RN Supervisor.
- Assists direct care professionals with ADLS and meals.
- Monitors meal preparation & feeding strategies ensuring meal plans and diet consistencies are followed in accordance with OPWDD guidelines and Choking Prevention Initiatives
- Perform Fit testing on medically cleared employees for N95 respirator masks.
- Performs Covid testing on Individuals as needed & oversee Covid self-testing on employees as needed.
- Additional responsibilities may be assigned based on program needs.
- Keeps the nursing office clean, to ensure a healthy, clean environment for participants and staff. Ensures various supplies, equipment and other material used in training are in adequate supply, are stored correctly and in good order.
- Maintains Infection Control Practices.
- Performs Fit Testing when needed on Residential house staff.
- Must cooperate and participate fully with Jawonio and/or Justice Center and OPWDD during an investigation or inspection, including making verbal and written statements as requested by the person(s) doing the investigation.
- Maintains orderly, safe working area and work practices.
Qualifications
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience: Associate degree in Practical Nursing plus 2 years of experience in a similar setting. Must hold valid licensure from New York State as a Licensed Practical Nurse (LPN).
Language Ability: Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to individuals and employees of the organization. Ability to read, analyze, and interpret general medical periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of employees, managers, consumers, and families.
Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

Supervisory Responsibilities: This job has no supervisory responsibilities.

Additional Requirements:
- Remain current in all NYS OPWDD and/or OMH and agency required trainings (BLS and others)
- Candidates must be cleared and maintain acceptable record under the NYS OPWDD and NYS OMH mandated criminal background check process.
- Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
