Primary Job Functions
Provide access to a wide range of services for individuals experiencing serious mental illness
including direct service as assigned by supervisor.
Properly assess need and devise individual service plan for each client.
Effectively implement written service plans to meet client goals and objectives.
Serves as a liaison between the client and various service providers to ensure continuity of care
and assist with weekly psychiatric clinic.
Develop and maintain adequately detailed and accurate client records.
Provide transportation to clients when necessary.
Provide accurate documentation in debriefing for weekly and monthly reports / meetings as
stipulated by program demand.
Responsible for maintaining detailed HUD information through HMIS (Homeless Management
Information System) as required through the federal grant.
Perform all administrative duties as required.
Minimum Qualifications :
Bachelor of Arts or Bachelor of Science in a human service related field. Two years of
experience working with individuals living with mental illness. Experience with working with
individuals experiencing homelessness.
Valid Alabama driver’s license and driving record acceptable to agency automobile insurance
carrier for inclusion on agency policy. Suitable automobile to permit the required travel. Must
show maintain car insurance equal to or in excess of the State of Alabama standards for
automobile insurance coverage.
This position requires completion of the State of Alabama Adult Serious Mental Illness case
management training.
Knowledge, Skills, Abilities :
Knowledge of mental illness, symptoms and treatment regimens.
Ability to work autonomously and make sound judgements regarding client care.
Knowledge of the special needs and behavioral characteristics of individuals experiencing
serious mental illness.
Knowledge of the community and available community resources.
Knowledge of the rules and regulations governing the human series provider agencies in the
catchment area.
Knowledge of the legal and ethical issues relative to confidentiality of client records and the
ability to interpret and apply them within the scope of the position.
Knowledge of casework principles and methods related to case management.
Working knowledge of interviewing techniques and principles.
Ability to communicate effectively with a multi-disciplinary team.
Working knowledge of developing needs assessments and case plans.
Ability to document activities and maintain records in a timely fashion.
Skills in interpersonal relationships and dynamics.
Considerable written and expressive communication skills.
Acquainted with the use and side effects of psychotropic medications.
Ability and willingness to work in area shelters, streets, boarding homes or foster homes, as well
as sites in which individuals experiencing homeless and serious mental illness may reside.
