Early Memory Loss Program Supervisor

Principal Responsibilities:

The Marion & Aaron Gural Jewish Community Center, a non-profit community agency located in the greater Five Towns is looking for a Project Supervisor for Early Memory Loss to work in our Adult Day Program. This position will report to the Director of Adult Engagement. The Early Memory Loss Program provides meaningful social and recreational activities in a secure and supportive setting. The goal is to maximize cognitive and social capabilities of people with dementia and provide caregivers respite, ongoing support and access to resources.

• This is an on-site role working Monday-Thursday, 28 hours a week.
  
  

Job Responsibilities:

• Promote and Recruit for this newly developed program
• Coordinate all aspects of the program activities including transportation
• Set up program room, order and serve lunch and snacks
• Plan and Chaperone trips
• Daily record keeping including clients' attendance and sign-in logs
• Ensure implementation of the agency's policies and practices
• Provide psycho-social client intakes and Person-Centered Care Plans
• Communicate with clients and families
• Provide communication and assist with contracts with the Long-Term Managed Care Agencies
• Provide an environment of dignity and respect for all clients
• Maintain safety of participants at all times
• Supervise Program Coordinator
• Other duties as assigned
  
  

Minimum Qualifications:

Job Requirements:

• MSW or LMSW with at least one year of experience working with Older Adults
• At least one year experience as a supervisor
• Experience with Alzheimer's/ Dementia
• Must be very familiar with Jewish customs and observance
• Ability to work with diverse teams and clients.
• Program Development and Outreach
• Strong time-management and organizational skills.
• Ability to Muli task and problem solve
• Strong verbal and written communication skills in English language.
• Must be able to move tables and chairs and lift 20lbs
