Position Summary: Child & Family Treatment & Support Services (CFTSS) and Home and Community Based Services (HCBS) are Medicaid funded services that are designed to allow children/youth ages 0-21 who are at risk of developing or have been diagnosed with behavioral, developmental or medical diagnoses to participate in a vast array of developmentally and culturally appropriate services in the home and in the community. The Service Providers (SPs) are primarily responsible for delivering quality CFTSS and/or HCBS services to eligible youth to support the achievement of safety, well-being and permanency objectives.

Responsibilities:

  •  Provide direct services with clients during after school hours and weekends as needed to meet client needs; responsible for meeting billable targets in an individual, family, collateral or group modality as follows: full-time providers -19-hours per week; part-time providers-11-hours per week.
  • Provide services based on family’s needs and requirements set by the governing agencies. Must be available to provide services via face to face and/or telehealth.
  • Attend supervision (weekly/bi-weekly with supervisor) as well as one monthly meeting with LCSW. Attend additional meetings as required
  • Provide services as prescribed on the treatment/service plan. Maintain assigned schedule; ensure frequency, scope and duration of services is met
  • Ensure progress notes and timesheets are completed within 48 hours of service completion in the EHR system; any required corrections noted by Supervisor must also be completed within 48-hours of receipt.
  • Complete monthly contact logs for over and under service utilization.
  • Complete contact logs for non-billable contacts.
  • Monitor the youth’s progress in achieving goals detailed in the treatment plan; collaborate with other care providers and community supports to help youth meet their needs
  • Update Treatment plans at least every 6 months but more often if needed
  • Collaborate with referral source as needed (HHCM/Foster Home/ACS. Upon receiving required HIPPA Consents). Review treatment/service plans with family and obtain signatures.
  • Create an initial Safety Plans within 30 days and update the safety plan as needed (minimum every six months)
  • Notify manager/supervisor immediately upon hearing of an incident.
  • Complete Incident reports as needed with 24 hours of being notified of the incident and contact the child abuse hotline when required.
  • Complete all training requirements within the required deadline.
  • Date of First Service (DOFS) form to be completed and submitted immediately following the initial contact.
  • SP has 48 hours to reach the family to introduce themselves and schedule initial service within 5 days of contacting the family.
  • Utilize JCCAs electronic health record, Electronic Visit Verification (EVV), Microsoft Outlook and other systems required to complete roles and responsibilities. 

Qualifications/Services:

  • Other Licensed Practitioner Provider (Counseling) - provides psychotherapy, family therapy assessments and referrals to other services in the home or in the community Requires licensed psychologist, psychoanalyst, Social Workers (LMSW or LCSW) LMHC, LMFT, Licensed Creative Arts.
  • Community Psychiatric Support & Treatment Provider - services works to develop child’s social and independent living skills, stabilize child and family after crisis and help caregivers respond in situations that could trigger a crisis.  Support services work with caregiver to plan child’s treatment educate child on appropriate safety and self-care, and connect to resources. Requires a minimum of a Bachelors in a helping profession and at least 3 years of experience in human services.        
  • Psychosocial Rehabilitation Provider - assist with study habits, coping skills, hygiene, daily living activities and socialization for a child with mental health needs they can address inappropriate behaviors, anger management and coping mechanisms. Requires a high school diploma and at least 2 years of experience in human services
  • Family Peer Support Services Provider The provider works with the caregivers to understand the child’s diagnosis and development. They can refer the caregiver to community resources and accompany them to appointments. Requires lived experience AND at a minimum, high school diploma, high school equivalency preferred or a State Education Commencement Credential.  Can be waived in certain situations with an attestation.       
  • Youth Peer Support - is between the ages of 21 to 30 and has a similar background to the client.  The provider models’ healthy choices, coping skills, self- advocacy and goal-setting for teens (ages 14 ) navigating the transition to adulthood. Requires lived experience AND at a minimum, high school diploma, high school equivalency preferred or a State Education Commencement Credential.  Can be waived in certain situations with an attestation.
  •  Planned Respite Provider – provides planned short-term relief for families’ caregivers that are needed to enhance the family caregiver ability to support the child’s disability and or health care issues. 
  • A high school diploma, high school equivalency preferred or a State Education Commencement Credential and 1 year of experience working with children/youth
  • Community Habilitation Provider- works one on one with developmentally disabled child to set goals and master skills they need at home and in the community including hygiene coping skills daily living activities like chores and budgeting getting around the community and socialization. Requires a high school equivalency with one year of related human service experience with the developmental disability population.
  • Prevocational Provider- helps teens 14 with topics such as proper work habits problem-solving resume preparation and interviewing. Requires a minimum of an Associate degree and one year of human service experience.
  • Supported Employment Provider- provides teens 14 with a job coach to assist in finding a job or vocational opportunities. Requires a minimum of an Associate degree and one year of human service experience.
  • Caregiver/Family Advocacy and Support Services - Assist the child/youth, family/caregiver, and collateral contacts in understanding and addressing the participant’s needs related to their disability(ies). These services can enhance the child/youth’s ability, regardless of disability (developmental, physical, and/or behavioral), to function as part of a caregiver/family unit and enhance the caregiver/family’s ability to care for the child/youth in the home and/or community. Requires a minimum of a high school diploma and 3 years of experience in related field and/or human services.

 Working Conditions/Physical Demands:

  • Ability to travel to multiple work sites via public transportation             
  • Repetitive movement of hands and fingers (typing and/or writing)
  • Ability to stand or walk long distances
  • Visual acuity to view documents on a computer screen for long periods of time
  • Typical office environment
  • Work in extreme heat or cold conditions
  • Flexibility to work evenings/nights/and-or weekend
  • Ability to travel to multiple work sites in NYC boroughs

Salary Range:  $26.00 to $35.00 per hour 

Location: Queens, New York

Status: Position is part-time (20) hours a week, schedule is flexible. Hours are available for afternoon, evening and/or weekends.
