Behavior Support Professional /Case Manager
The Jackson County Developmental Center of WV is hiring a dual-title position as a Behavioral support professional 1/ Case Manager for the IDD/Waiver Program at our Jackson and Wood County locations. The IDD/Waiver Program provides services to people diagnosed with an intellectual disability. This position works under the direct supervision of the Director of Waiver Services, is a part of the JCDC Clinical Team, and is responsible for assisting in implementing the mission of JCDC.
Case Managers are responsible for the identification of Medicaid Waiver members’ individual needs, including physical and mental health, social services, vocational services, and long-term support services. Case Managers will also be responsible for developing the member’s Individualized Service Plan of Treatment (IPP) to address needs. Case Managers ensure timely and coordinated access to an array of providers and services and develop solutions to address the unique needs of individuals and coordinate services delivered as necessary and appropriate.
Behavioral support professional 1 is responsible for ensuring person-centered supports are provided as outlined by each individual's treatment team. The position requires using independent judgment and accepting responsibility for actions. This person develops training programs and desires to help provide a safe, nurturing environment where participants can thrive.
This position is conflict-free. The individual selected will work with a caseload that includes a specific group of individuals receiving Behavioral Support services, while the remaining clients will benefit from case management services. This role is designed to meet the needs of individuals based on the services

What you will need:
· A bachelor’s degree is required from an accredited college or university in nursing, social work, counseling, special education or a closely related field of study.
· Preferred: A minimum of one year of experience working with individuals who are disabled.
· Social Work License or eligibility preferred.
· Demonstrated ability to work with clients and families.
· Valid driver’s license and automobile insurance coverage.
· Criminal Background Check.
· Flexibility to work irregular hours on occasion, including evenings and weekends.
· Available to be on Call
