GENERAL SUMMARY OF DUTIES : Licensed Professional Counselor (LPC) / Licensed Clinical Social Worker (LCSW) will provide Behavioral Health services to all assigned patients. LPC / LCSW will work closely with BH Director and BH Coordinator to ensure Best Practice techniques are utilized when addressing the needs of patients. LPC / LCSW will assess and monitor patents' progress by adhering to agreed upon treatment plan. LPC / LCSW will maintain efficient record keeping and clinical documentation via Electronic Medical Record. LPC / LCSW will participate in all clinical staff meeting by presenting any problematic case that needs group consultation. SUPERVISION EXERCISED : None ESSENTIAL FUNCTIONS :
- provide counseling services to assigned patients.
- perform initial mental health evaluations resulting in realistic treatment plans
- assessing patients using standardized assessment / screening tools or measures
- provide brief, solution focused therapy
- provide group psychoeducation / treatment related to chronic medical conditions
- provide information and referral services
- support care coordination
- perform crisis assessments / triage with appropriate interventions
- provision of other services related to wellness
- ensure proper documentation of services provided in accordance with requirements in the electronic health record.
- provide other related activities designed to provide integrated health care services
- provide individual psychotherapy with children, adults, and families
- assist and support scheduling in electronic health record
- provide psychotherapy to children and adultEDUCATION / EXPERIENCE : LPC / LCSW with a master's degree from an accredited university A minimum of three (3) years of job-related experience, preferably in a Community Health Care Clinic environment with the responsibility of providing behavior health counseling services. LANGUAGE SKILLS : Ability to read, write, speak, and comprehend written documents fluently.CERTIFICATES, LICENSES, REGISTRATIONS : In good standing with Louisiana State Licensure Board. Proof of professional certification, licenses or registration required. All candidates will be required to submit to a criminal background check and screening as part of pre-employment.PHYSICAL DEMANDS : This role requires the ability to lift files, open filing cabinets and bend or stand on a stool, as necessary. This position requires the ability to occasionally lift office products and supplies up to twenty pounds.
