Direct Care Worker
Duties:
Responsibilities:
- Implement and follow care plans for individuals under your care
- assist with daily hands on support - house/home skills
- Assist with case management, including coordinating appointments and communicating with healthcare professionals
- Maintain confidentiality and adhere to HIPAA regulations when handling sensitive information
- assist with and ADL skills
- Administer first aid when necessary
Qualifications:
- Previous experience in direct care or a related field is preferred
- Familiarity with care systems and case management processes
- Ability to effectively communicate and collaborate with healthcare professionals, clients, and their families
- Strong attention to detail and ability to accurately record information
- First aid certification or willingness to obtain certification
