Job Title: Designated Coordinator
Reports to: Designated Management
Department: IHS/ILS
Job Summary:
The designated coordinator at a 245D company oversees and coordinates services for individuals with developmental disabilities. Responsibilities include ensuring compliance with regulations, creating individualized service plans, and collaborating with staff and external partners to enhance the overall well-being of individuals served.
Duties and responsibilities:
- Train and manage assigned staff members according to recipients’ needs and goals
- Maintain compliance with federal and state standards and laws
- Coordinate meetings between staff, individuals, case managers, and guardians
- Gather current and accurate information in order to support the individuals served needs and goals
- Accurately complete and update necessary paperwork
- Communicate frequently with guardians, management, and caseworkers
- Attend, assist, and engage in team meetings as needed
- Maintain an average of 60% (24 hours) individuals served contact time and 40% (16 hours) administrative time average weekly
- Maintain and complete continuing education as required
- Reporting suspected or known abuse according to company policy
- Uphold confidentiality and privacy of individuals served and staff members
- Other duties as assigned
Required Skills/Abilities:
- Ability to write, understand, and implement a variety of Community Support Plans and Individual Service Plans
- Ability to track and chart data on a daily basis in compliance with company expectations
- Ability and willingness to fill in House Coordinator and Behavior Technician roles as needed
- Dynamic communication skills, working techniques, and ability to multitask
- Ability to work independently
- Fluently read, understand, speak, and write the English Language
- General computer skills/knowledge
- Monthly trainings as assigned
Qualifications:
- Must be at least 18 years of age and possess a High School Diploma, GED, or equivalent, one of the following below is required:
- BA or 1 year full time experience providing services to individuals with disabilities
- AA or 2 years full time experience providing services to individuals with disabilities
- Diploma in a field related to human services from an accredited postsecondary institution 3 years full time experience providing services to individuals with disabilities
- 4 years full time experience providing services to individuals with disabilities
- Ability to pass a background check with the MN Department of Human Services
- Knowledge of federal and state healthcare laws and regulations
- Previous training and management experience
- Willingness to travel to and from surrounding area communities as needed
- Reliable transportation and a current driver’s license
- Clear driving record (No major violations or DUI/DWI convictions in the past 5 years)
- Valid proof of auto insurance per the state of Minnesota
- Working cell phone with internet access
Preferred Qualifications:
- Associate’s degree, or higher, in social work, human services, or similar studies
- Current CPR and First Aid Certification
- Strong 245D Familiarity
- Multilingual - fluent, ability to translate if necessary

Pay starting at $22 an hour based on experience
