About the Job:
Impact Physician Group is an established physician services organization providing services to a full spectrum of post-acute and specialty facilities. The group provides internal medicine physicians, advanced nurse practitioners and physician assistants who focus on individualized care and coordinating specialty physician services in collaboration with therapists, nurses, and pharmacists.

Supervisory Responsibilities:
· None.
Duties/Responsibilities:
· Perform evaluation and management services for inpatient acute physical rehabilitation requiring cardiopulmonary management services.
· Admits patients and completes all related documentation.
· Initiates plan of care for newly admitted patients.
· Perform dictation of H&P examinations.
· Does related follow up for patient issues and cross-coverage for the hospitalist team.
· Collaborate with hospitalist and providers in developing and providing cardiopulmonary management.
· Is alert to any changes in condition and initiates early management to sustain stay and cause a return to a higher level of care to be unnecessary.
· Evaluates major change in condition promptly and reports to hospitalist.
· Work closely with the therapy and nursing team to assure strong interdisciplinary plan of care.
· Perform such other responsibilities as reasonably assigned by the chief hospitalist and with the NP/PA scope of practice.

Education and Experience:
· Successful completion of an approved nurse practitioner or physician assistant program.
· Must have a current, unrestricted advanced practice licensee in the state of Indiana.
· 1-2 years of recent hospital experience preferred
Physical Requirements:
· Requires the ability to sit at a desk for period of time and intermittently walk, stand, stoop, kneel, crouch, and reach while performing office duties.
· Life and or move light weights
· Use hands to finger, handles or feel objects, tools, or controls.
· Must be able to maintain effective audio, visual discrimination and perception needed for making observations, communicating with others, reading, and writing, and operating office equipment.
· Must be able to use telephone to communicate verbally and a keyboard to communicate through written means, to review information and enter/retrieve data, to see and read characters on computer screen.
· Requires the strength and stamina to perform clinical duties.
· May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.

Salary Information:
Factors used in determining the salary offered include the candidate’s qualifications as well as the pay rates of other employees based on the salary surveys for similar positions within the area.

Additional Job-Related Information:
· Daily production requirements on average of 20-25 per day.
