Jefferson Center is seeking a Clinical Program Coordinator to join our recovery oriented, phase-based Adult Collaborative Care Clinic. Our phase-based care model will help people thrive by continuously revolutionizing healthcare delivery, using a data-driven and team-based approach.
Phase-based care is an interdisciplinary treatment approach rooted team collaboration, data, and measurement, where each member contributes their expertise. Caseloads are shared among the team, which is comprised of a psychiatric prescriber, nurse, therapists, care coordinators, peer specialists, and program support staff. The latest team-based clinical practices and tools are employed to help those in need get better fast and maintain their health and wellbeing long-term.
We seek to offer our community access to the best behavioral health and wellness services starting on the same day they are requested, without exception. We will achieve this while simultaneously creating a work environment where our team has the autonomy, support, and tools to be wildly effective, satisfied, and engaged at work.

Essential Duties
For each phase-based care team, the responsibilities of a Program Coordinator include but are not limited to:
- Hire and maintain all assigned staff including oversight of timesheets and schedules to ensure adequate coverage for service provision
- Provide clinical and administrative supervision to ensure that the day-to-day operations comply with all regulatory requirements
- Provide evidence-based psychotherapy for clients within an interdisciplinary team of administrative staff, peer specialists, case managers, and medical providers, as needed to supplement and support clinic operations
- Ensure compliance with policies, procedures & regulations; maintain quality of records
- Coordinate sufficient Leadership on Site (LOS) coverage for the Lakewood and Evergreen locations on the days the clinic operates in person.
- Respond to and manages any crisis concerning clients and/or potential clients
- Consult with clinicians/staff on high acuity cases and make appropriate referrals for higher levels of care or other services as necessary.
- Develop and monitor approaches to improve client engagement in needed ongoing services and utilization of internal/external resources.
- Share knowledge of recovery-based, stages-of-change, motivational interviewing, cognitive-behavioral, solution-focused, trauma informed and culturally competent treatment. Ensure staff adhere to the evidence-based therapeutic and psychoeducational programing
- Regularly consume and utilize outcome data to drive clinical collaboration discussions and consequently, a client’s care planning
- Exhibit enthusiasm, courtesy, adaptability, and spirit of cooperation and teamwork in the work environment
- Initiate and maintain positive interactions, timely response to messages and other requests
- Utilize data and new operational practices to better adapt caseload complexity
- Coordination for emergency services when clinically indicated or medically necessary; risk/lethality assessment
- Plan, direct and facilitate team meetings/consultations as assigned
- Provide training on initial assessment, ongoing therapy, and case management services as well as overall procedures and paperwork
- Review and prepare charts for the annual Site Visit
- Review and approve charts and treatment plans for clinicians ensuring all documentation standards are met in the electronic health record; ensure staff meet 90% of all Progress-to-Date forms within 3 working days and completes 95% of all paperwork by the due date
- Ensure that fee collections meet the team goal as determined by Network Director
- Complete performance evaluations, employee contribution discussions, stay interviews, and any other required evaluations at interval set by Jefferson Center.
- Monitor on-going performance measure areas such as consumer service hours, paperwork and customer service expectations with assigned staff including developing action plans and disciplinary measures as appropriate.

Education, Knowledge, Skills And Experience Required
- Licensure (LCSW, LPC, LMFT, or LP) required. Master's Degree required.
- Four or more years related experience and/or training required; strong assessment skills and experience working with individuals with mental health issues required.
- Skills in a robust set of clinical interventions required; preferred particularly in areas of crisis interventions, culturally competent interventions, cognitive behavioral therapy, play therapy, and other applicable treatment modalities
- Knowledge and understanding of the effects of trauma on individuals who access services required
- Knowledge of stages of change, motivational interviewing, culturally appropriate episode of care and trauma informed care practices are required
- Prior clinical supervisory experience preferred.
- Ability and willingness to work in face-to face and via video telehealth
- Skills, experience, or vested interest in integrated health care approaches
- Comfort with or willingness to quickly learn various technologies
- Bilingual preferred but not required
Additional Salary Information*:
- The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
Salary : $67,300 - $85,700
