Brief Description of the Phase Based Teams:
Jefferson Center is hiring for the Team Lead position specific to the clinic below:
Recovery Collaborative Care Clinic
The Recovery Collaborative Care Clinic provides immediate, non-crisis support and therapeutic services for adults 18 and older who have a primary substance use diagnosis or a co-occurring diagnosis with Medicaid coverage for therapeutic trauma-informed services. The Recovery Clinic provides wraparound clinical services from care coordination, initial and concurrent assessments, individual therapy, case management and medication management services. The Recovery Clinic can also provide support to their significant caregivers and or families.
Education, Knowledge, Skills & Experience:
- Master’s degree in social work, counseling, psychology, or another related field
- LPC, LCSW, or LMFT. CAS, or LAC is required
- Skills, experience, or vested interest in integrated health care approaches
- Willingness to provide care both face-to-face and via telehealth (video)
- Comfort with or willingness to quickly learn various technologies
- A minimum of 2 years experience working with the Substance Use Disordered population required
- A minimum of 6 months experience providing clinical supervision and/or serving in a leadership position preferred
Essential Duties:
For each phase-based care team, the responsibilities of a Team Lead include but are not limited to:
- Assist with supervision of master's level interns or other staff depending on team/Program Manager need.
- Participate and assist in onboarding new team members/interns by training, providing shadowing opportunities, and acting in a mentor capacity. Provide on going training for existing staff.
- Provide support to the Manager of Recovery Clinic with team operations such as approving and reviewing chart documentation and referrals, attending meetings including weekly Recovery Clinic team meetings, participating in new hire interviews, and auditing.
- Provide therapy and case management services to consumers including service planning, monitoring, follow-up, linkage, and resource referrals.
- Provide immediate crisis evaluation and support to consumers who walk in and need crisis support. Assess clients for risk/lethality and coordination with crisis or emergency services when clinically indicated/medically necessary, including hospital admissions and M-1 holds.
- Make appropriate referrals to and act as liaison with community agencies, other service providers, and natural support systems for consumers on assigned caseload
- Collaborate with consumers, family members, and other supports in identifying strengths, cultural factors, and recovery needs for developing personalized treatment goals
- Knowledge of Stages of Change model, Motivational Interviewing techniques, risk assessments, Person Centered treatment approaches, Trauma Informed care, Cognitive Behavioral Therapy skills, and Recovery-Based Treatment practices is essential. Knowledge of DBT skills and models of clinical supervision helpful but not required.
- Provide assistance in developing or connecting team and consumers to resources for developing social, vocational, spiritual, cultural, and leisure abilities as well as basic needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric services.
- Maintain communication and care coordination with other Jefferson Center programs.
- Engage in ongoing monitoring of consumers on assigned caseload including assessment of symptoms to prevent relapse, assessment of daily living skills (ADL’s), engagement outreach, hospital discharge planning, substance use assessment, and respite service needs.
- Follow ethical and clinical practice guidelines as well as Center and State standards to provide appropriate service delivery and maintain standards of care.
- Assist with special projects or Center committees as indicated, desired, or assigned.
Other Duties:

- Participate in supervision by coming prepared with an agenda. Report high risk/problem cases, and utilizes a problem solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor.
- Submit progress forms within 3 working days.
- Complete documentation by the due date.
- Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.
- Is aware of client fees and balances and collects fees and develops payment plans in a timely fashion to ensure that fee collections meet the team goal as determined by Recovery Clinic Director.
- Provide up to date information on caseload status when requested.
- Effectively respond to client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, pages, email and other requests.
- Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteer to work on Center committees and/or task forces.

Salary Grade 52: $70,000 - $77,200 annually* (Plus an additional 5% for team lead duties)
Additional Salary Information*:
- The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
