Jefferson Center is seeking a therapist to join our recovery oriented, phase-based care team. The Adult Services Collaborative Care Clinic provides immediate, non-crisis support and therapeutic services for adults 18 and older and their significant caregivers / families. The Adult Services Collaborative Care team provides wraparound clinical services from care coordination, initial and concurrent assessments, individual therapy, case management, and medication management services. Our phase-based care model will help people thrive by continuously revolutionizing healthcare delivery, using a data-driven and team-based approach.

Education, Knowledge, Skills & Experience Required :

  • Master's degree (from accredited University / College) in social work, counseling, psychology, or another related field required.
  • Independent clinical licensure (LCSW, LPC, LP, or LMFT) strongly preferred.
  • Skills, experience, or vested interest in integrated health care approaches
  • Willingness to provide care both face-to-face and via telehealth (video)
  • Comfort with or willingness to quickly learn various technologies
  • Willingness and ability to work in a shared and collaborative environment

Collaborative and Phase-Based Care

Our clinic utilizes an interdisciplinary treatment approach rooted in team collaboration, data, and measurement. The clinic team is comprised of peer specialists, care managers, nurses, prescribers, wellness coaches, administrative support staff, and therapists; each team member contributes their expertise. The latest team-based clinical practices and tools are employed to help those in need get better fast and maintain their health and wellbeing long-term. In addition, our team is also supported through this model by offering real time insights, support, and expertise to inform each session and help us determine what providers are appropriate during each visit.

  • Influence a dramatic reduction in wait times for accessing care and wait times between interventions in the initial phase of care
  • Provide continued services for each client's level of need(s) during their duration of care with the clinic
  • Increase our resilience to industry-wide staffing shortages and record high community demand
  • Decrease staff burnout via collaborative care staffing and availability
  • What does the day-to-day look like in this program?

  • Walk in services, with flexibility to provide some scheduled appointments
  • Team of equals - regular touch points and consultation
  • Provide excellent customer service
  • Generate new ways to help people get better faster
  • Use tech to analyze trends
  • Use tech to outreach and engage clients
  • Participate in team operation workgroups to advocate for program development opportunities
  • Essential Duties :

    For Collaborative Care teams, the responsibilities of a therapist include, but are not limited to :

  • Ability to provide services at the Evergreen office 2-4 times per month
  • Provide evidence-based psychotherapy for clients within an interdisciplinary team of administrative staff, peer specialists, case managers, health and wellness coaches, and medical providers
  • Administer evidence-based measurement tools regularly and utilize outcome data to drive clinical collaboration discussions and a client's care planning accordingly
  • Engage client and care team in strength-based and trauma-informed treatment and goal setting, and work constructively to reach agreed upon outcomes
  • Identify risk factors such as suicidality, homicidal ideation, and / or grave disability and coordinate with emergency services as necessary
  • Participate in evidence-based practices and other agency initiatives for mental health and co-occurring substance use services (Motivational Interviewing, Trauma Informed Care, CBT, DBT, etc.)
  • With considerable autonomy when making data informed treatment and programming decisions, exhibit your ability to adapt quickly to needs
  • Exhibit enthusiasm, courtesy, adaptability, and spirit of cooperation and teamwork in the work environment
  • Effectively respond to client needs and problems; initiate and maintain positive interactions; timely response to messages and other requests
  • Utilize data and new operational practices to better adapt caseload complexity
  • Engage in clinical supervision with the program coordinator. Participates in supervision by coming prepared with an agenda. Report high risk / problem cases and utilize a problem-solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor.
  • Provide clinical consultation to team staff and clinic supervision to unlicensed staff (as needed)
  • Have an opportunity to supervise an intern for their practicum, internship, or both
  • Consistently meets the expectations of the job. Corporate Compliance including documentation on practice in accordance with regulatory requirements and clinical guidelines, including submission of 90% of session notes within 3 days of service, completing 95% of paperwork by due date, and closing 60% of cases within 90 days after last contact.
  • Ensure that delivery practices are guided by the principles of trauma-informed care.

    Salary Grade 51 : $57,000 - $64,000 annually

    Salary Grade 52 : $70,000 - $77,200 annually

    Additional Salary Information

  • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours / week will be prorated and adjusted to the appropriate FTE.

    Salary : $57,000 - $64,000
