What are the responsibilities and job description for the Diabetes Care and Education Specialist (Certified) position at Jefferson County Health Center?
POSITION OVERVIEW:
Coordinates the Diabetes Self-Management Education and Support (DSMES) Program. Plans and implements education for the diabetes education program for patients and staff. This is done in conjunction with physician(s), nurses, and departments, for those patients with appropriate diagnosis.
QUALIFICATIONS:
· Graduate of an accredited school for Nursing, Pharmacy or Dietitian
· Bachelor’s Degree preferred
· Must be certified in Diabetes Education or willing to complete a professional diabetes management program within two years of hire. Must obtain CDCES (Certified Diabetes Care & Education Specialists) within 1 year of completing professional diabetes management program.
· Must maintain annual participation in continuing education in diabetes management as required for ADA recognition and CDCES status.
· Specialized skills to include, but not limited to, diabetes related signs and symptoms, treatment protocols, and management techniques highly preferred.
· Current Basic Cardiac Life Support (BCLS) certification
· Experience in a physician’s office preferred
· EPIC experience preferred
