What are the responsibilities and job description for the EMPLOYEE HEALTH & CASE MANAGER RN (Dual Role) position at Jefferson County Health Center?
POSITION OVERVIEW:
The Employee Health Nurse role is responsible for employee health for all new hires and routine assessments for current employees. Will be responsible for ensuring all staff are compliant with immunizations that meet the current policies of JCHC, CDC, and OSHA for new hires and current employees. Accountable and responsible for managing and maintaining compliance with new hires and current employees. Ensures compliance and tracking for all volunteers, outreach clinic staff, and contracted staff. The Employee Health Nurse will track and instruct staff regarding post-exposure needle sticks/other potentially infectious material exposures.
The Case Manager role will be responsible for assisting patients, families, and/or caregivers in giving psychosocial support, guidance, education, recommendations, and referrals for other services. Other duties they perform include providing case management or interventions to enhance health, avert disease and address obstacles to accessing good healthcare.
QUALIFICATIONS:
- Graduate of an accredited school of nursing, with current Iowa RN license
- Bachelor of Science in Nursing preferred
- BCLS required
- Experience as Employee Health Nurse or Case Manager RN preferred

POSITION-SPECIFIC REQUIREMENTS:
Employee Health RN Role
- Performs health assessments, tuberculosis assessment, and vaccine requirements for all new employees, volunteers and at times contracted staff prior to employment.
- Education to new and current employees of wellness benefits available to JCHC employees.
- Coordinates with contracted staff and outreach clinics for documentation of tuberculosis and vaccine requirements prior to starting at JCHC.
- Educate employees on healthy habits to improve their health.
- Maintains compliance with employee health records for employees, volunteers, contracted staff, locums, and outreach clinic’s health records in a secure location.
- Assists with planning and administration of yearly influenza vaccinations for staff.
- Assist staff with post-exposure needle sticks and other potentially infectious material.
- Other duties as assigned.
Immunizations
- Assist with completion of requirements for vaccinations for all new and current employees (including exemptions and declinations).
Employee Illness
- Follows up on employee illness reports.
- Review and evaluate absenteeism reports due to illness for all employees.
- Works with payroll on absenteeism
- Follows up with employee regarding return to work.
- Collaborate with Infection Preventionist as needed.
- Provides reporting for quality purposes.
Workers' Compensation
- Operates as safety officer and therefore, is responsible for OSHA requirements for employee injury and illness reporting.
- Works directly with the Worker’s Compensation corporate team to determine employee injury and prevention measures.
- Investigate employee injuries and illness with the employee, Manager/Supervisor, Human Resources, Quality Improvement, and additional staff as needed.
- Coordinates claims, including OSHA tracking, reporting requirements, and communication with the insurance carrier, employees, managers, and providers.
- Responsible for monitoring and coordinating follow-up treatment, job transfers, restrictions, exposures, and return to work. Seeks counsel from HR Director when direction needed.
- Oversees compliance that all employee injuries are recorded and documented.
- Works as a liaison between Worker’s Compensation and management to ensure that safe work requirements are met.
- Works closely with HR and Worker’s Compensation carrier when legal compliance concerns
Case Management RN Role
- Obtains social history to assess patient needs and develop strategies to assist patients.
- Acts as a resource within the organization and the community regarding health, social, food insecurity, placement, and other patient issues.
- Assist patients with securing funding for medical care as needed. Refers to financial assistance as needed.
- Must have knowledge of healthcare practice, quality improvement concepts, patient/family education concepts and theories.
- Must have knowledge of community resources and reimbursement mechanisms.
- Conduct face-to-face visits with patients at JCHC.
- Self-motivated with ability to work with team. Effectively communicates, problem-solves, and maintains productive and effective interpersonal relationships.
- Collects, organizes, documents, and analyzes data, synthesizing it into understandable information
- Works with outside facilities and agencies on a routine basis, maintaining positive working relationships.
- Must have exceptional customer service supporting patients, families and caregivers while providing compassionate care.
- Assist with Mental Health and Substance Abuse committal process.
- Coordinates continuity of care by connecting patients with community and other resources.
- Ability to communicate with small and large groups.
- Attend ongoing training and courses to keep abreast of new developments in health care.
- Other duties as assigned.
Budget
- Collaborates with the Quality and Infection Control Director with annual budget as related to employee health.

- Collaborates with Human Resources, Safety committee, and Leadership team for adjustments and changes for employee wellness needs.
REPORTING REQUIREMENTS/RESPONSIBLITIES:
- Monthly-Patient Safety-Employee illness report
- Bimonthly: Employee illness and injury report for Medical Staff quality
- Quarterly: Quality Improvement reporting as determined by Quality Improvement Department.
- Annually:
- Complete OSHA reporting to include 300A posting and reporting requirements to OSHA website (complete: January, Internal Posting: February 1st - April 30th).
- NHSN-Influenza Vaccination to Quality Improvement (monthly October-March)
