Position Title: Care Partner/Resident Assistant
Department: Nursing
Reports To: Resident Care Coordinator, Jennings at Brecksville
Hours: Second shift, Part Time hours available - every other weekend on 2nd shift.
Position Summary:
(Overall purpose of this position) This position provides personal care and basic nursing tasks as outlined in the State Tested Nursing Assistant Training Program to assure the safety, comfort and well-being of all residents.

- Follow established performance standards and perform duties according to nursing service policies and procedures.
- Identify special resident problems and/or change in resident's condition and report to the Neighborhood Coordinator or Resident Care Coordinator.
- Provide direct resident care as assigned.
- Conduct resident rounds daily at change of shift with on/off going staff.
- Identify and inform the Neighborhood Coordinator or Resident Care Coordinator of safety hazards and emergency situations and assist in implementation of corrective action.
- Participate in education programs as assigned; must attend programs totaling a minimum of 12 hours per calendar year.
- Assume personal responsibility for following Jennings at Brecksville procedures related to control of equipment and supplies within the neighborhood. Keep Personal Care Caddies clean and adequately stocked; keep chemicals in locked storage when not in use.
- Initiate quality monitors and assume accountability for compliance with federal, state and local regulations within the neighborhood assigned and within his/her span of control.
- Report unusual problems or incidents to Neighborhood Coordinator or Resident Care Coordinator.
- Document incidents and/or unusual problems according to established facility procedures.
- Participate in resident care conferences and other meetings as directed.
- Review Activities of Daily Living directives and provide care to residents assigned as outlined.
- Communicate to the Neighborhood Coordinator or Resident Care Coordinator the resident's response or lack of response to care provided and, when appropriate, document such information. (example: resident refuses to take bath).
- Perform basic nursing skills as outlined in the State Approved Nursing Assistant Training Program.
- Follow Jennings procedures for admission, discharge, and transfer of residents.
- Listen to resident and family complaints and report problems to Neighborhood Coordinator or Resident Care Coordinator.
- Answer call lights in a timely manner.
- Serve meal trays on a timely basis according to Jennings policy and assist in feeding residents.
- Provide care for incontinent residents and utilize disposable incontinence pads correctly, per facility policy and procedure.
- Escort and help ambulatory residents walk as appropriate or assigned.
- Follow individualized plan for each resident assigned and as designated in plan of care.
- Make sure all residents are offered nourishments at designated times and assists with feeding when necessary.
- Complete tub baths, showers or bed baths; operate whirlpool bath.
- Turn and reposition all residents every two hours either in their beds or chairs, if care plan requires.
- Place bedpans, linen, washbasins and soiled linen in proper area.
- Complete food percentage, intake and output, Bath list, BP&WT list, ADL directive sign off and other documentation as assigned.
- Empty and record all urinary drainage units. Provide Foley Catheter care.
- Weigh resident accurately and record in designated area, as well as discrepancies to Neighborhood Coordinator or Resident Care Coordinator.
- Take resident's blood pressure accurately and record.
- Take oral and rectal temperatures accurately and record.
- Provide and/or assist the resident with personal care: shampoo, shave, mouth care, denture care, nail care, preventative skin care, putting on and taking off clothing.
- Fill water pitchers and distribute.
- Transport residents to activities within the Home including Chapel, arts and crafts, therapy, beauty shop.
- Transfer residents using proper body mechanics and assistance. Using Hoyer lift when designated and per policy.
- Provide social stimulation and interaction with the resident via conversation and involvement with ADL’s. Promote a cheerful, homelike environment for residents and show courtesy and respect for families and visitors.
- Perform housekeeping tasks as assigned; keep rooms and work areas neat and orderly. Change and wash residents’ linens and personal clothing as assigned.
- Maintain confidentiality of all information regarding residents.
- Consistently work cooperatively with residents, nurses, staff or other departments, restorative aides, other nursing assistants, physicians, families, consultant personnel, and auxiliary service providers.
- Come to work in a clean, neat uniform and consistently present a professional appearance.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Assist new staff members in following established facility policies and procedures.

Qualifications
- High school diploma or STNA license
- Basic reading, writing, and arithmetic skills
- Mature and compassionate personality
- Good communication skills. Understands and communicates job-related information and directions in English.
