SUPERVISOR : Director of Nursing
PRIMARY LOCATION : Barton, Breckenridge
HOURS : M-F 8-5, Some Sat. 9-12, Flexible (May include weekend and / or evening hours as needed)
CLASSIFICATION : Full time / Hourly / Non-Exempt

QUALIFICATIONS :
- Graduation from an accredited nursing program with a current NY state licensure as a Registered Nurse
- Direct experience in a primary care (preferred but not required)
- Reliable transportation between sites required.
- Current CPR and Basic Life Support certification (BLS)
- Maintain credentials, attend seminars, and fulfill all licensure requirements including continuing education.
- Demonstrated success in providing clinical care and nursing support with a solid knowledge base of medical practices and procedures within the primary care environment.
- Demonstrated ability and familiarity with tools, technology, and systems found within health care environments (such as Basic Computer Skills, Electronic Medical Records, Experience with Medent EMR a plus).
JOB SUMMARY : The Nurse Manager will model and educate their team on how to provide relevant nursing support services in a primary care setting to patients. This team leader will provide supervision and oversight for nursing staff at the Barton and Breckenridge locations. They will manage escalated patient issues and nursing staff concerns, demonstrating care and candor and leadership excellence.
Essential Job Duties
PATIENT CARE
Collaborate to improve patient outcomes and provide quality careEnsure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state / local regulations, and patient satisfaction.Perform physical exams and patient assessments to identify and address health problems and unmet patient care needs.Triage patient concerns and immediate needs by phone or in person.Provide nursing intervention and health education for individuals, families, and / or other groups regarding disease prevention, childbirth, support programs, and other health-related concerns.Perform medical treatments (medication and vaccine administration) as prescribed by a medical provider or by standing ordersCollect samples (blood, urine, stool, or sputum) from patients and perform routine laboratory tests.Prepare patients for examination, testing or treatments and explain procedures.Perform EKG and SpirometryHave a working knowledge of how to operate within the Medent (EMR) system, healthcare regulations and compliance within the healthcare system, Department of Health and HRSA (Health Resources and Services Administration) standards.Collaborate with Providers to develop business strategies and patient servicesModel, monitor and hold staff accountable to all HIPAA regulations and guidelines.Collaborate and establish efficiencies with the nursing leadership team to identify, implement and manage all aspects of excellent patient care with high standardsSupport implementation of major clinical initiatives for community wide distribution such as testing, vaccinations and support population health initiativesMonitor nursing team under your supervision and hold the team accountable for proper processes and care of patientsEnsure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state / local regulations, and patient satisfaction.Demonstrate knowledge of current standards of practice and perform current nursing practice expectations within established guidelinesMaintains current certificationParticipate in Quality Improvement initiativesSUPERVISION

TRAVEL REQUIREMENTS : Must be able to travel to and between sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
