Description
We are seeking a qualified Nurse Case Manager to support the U.S. Navy Special Operations Forces under the Preservation of the Force and Family (POTFF) program at Dam Neck, Virginia. The Case Manager will be responsible for delivering comprehensive, patient-centered care coordination to Department of Defense beneficiaries. This role focuses on the development and management of cost-effective, evidence-based treatment plans, contributing to the overall readiness and resilience of military personnel and their families.
Responsibilities
• Conduct and document comprehensive health assessments, including detailed medical histories
• Develop time-sensitive, individualized treatment plans based on clinical findings
• Assess patients telephonically using established protocols to ensure appropriate care
• Monitor patient progress toward care goals and adjust plans as needed
• Oversee discharge planning to coordinate follow-up care and services
• Facilitate referrals to appropriate healthcare providers and community resources
• Negotiate and administer service agreements to support care delivery
• Maintain active credentialing and clinical privileges at the local Military Treatment Facility (MTF)
• Collaborate with internal and external healthcare professionals and stakeholders
Requirements
Minimum Qualifications
• Active, unrestricted license as a Registered Nurse in any U.S. state, District of Columbia, or U.S. territory
• Minimum of two (2) years of professional nursing experience
• At least two (2) years of experience in nurse case management
• Certification as a Case Manager (CCM or ANCC), or eligibility to obtain certification within one (1) year of hire
• Current Basic Life Support (BLS) certification through the American Heart Association
• Proficiency in interpreting medical diagnoses, managing complex patient cases, and coordinating interdisciplinary care
Clearance Required: Ability to obtain and maintain a SECRET clearance

Required Knowledge and Skills
• Strong verbal and written communication skills
• Ability to coordinate with multiple teams and facilitate care across systems
• Knowledge of VA/DoD Clinical Practice Guidelines and standardized utilization review criteria
• Familiarity with TRICARE, Medicare, and other third-party payer systems
• Experience with health information systems, data tracking, and outcome reporting
• Understanding of pharmaceuticals, including their effects and interactions
• Knowledge of federal healthcare regulations, professional ethics, and patient benefits eligibility
Preferred Experience
• Prior experience working with military or veteran populations
• Understanding of the Military Health System (MHS) and POTFF program goals
• Familiarity with community healthcare resources and transition of care planning
