What are the responsibilities and job description for the Direct Support Professional- Old Colony position at JESSICA AND FRIENDS COMMUNITY?
Definition and Basic Function: Will be involved in the general supervision, care, and activities for the Individuals in the home and/or in the community in keeping with the mission, ministry, and Core Values of Jessica & Friends Community.
Essential Responsibilities:
Programming
1. Provide age and functionally-appropriate program activities that include assistance, training, and support for the acquisition of functional skills, personal needs, communication, and personal adjustment to promote greater independence and participation in the community.
2. Implement goal plans as directed and assist in developing goal plans.
3. Direct communication of important information to supervisory staff.
4. Correctly and accurately record individual’s activities and progress on case notes, goal plans, behavior plans, community outing sheets, and other forms as required.
5. Meet all the documentation requirements of the Electronic Visit Verification regulations.
6. Provide individuals with opportunities for community integration.
Food and Housekeeping
1. Assist the individuals in food preparation and in kitchen duties, teaching appropriate skills.
2. Prepare meals when necessary following nutritional requirements and using proper food storage guidelines.
3. Assist and monitor with eating and follow eating plans e.g., choking.
4. Assist the individuals with in-room care and general house cleaning, teaching appropriate skills.
Personal Care
Assist individuals with personal care and supervision, as outlined in their Individual Support Plan, which may include, but not limited to:
a. Bathing
b. Toileting
c. Grooming
d. Dressing
e. Eating meals
Medical
1. Report medical observations and concerns to the family, the Home & Community Program Manager, and/or the Home & Community Assistant Program Manager.
2. Document seizures, menses, incidents, etc. on appropriate forms.
3. Administer medications to the individuals as prescribed following organizational protocols after becoming trained in medication administration,
4. Become familiar with and receive training in specific medical protocols, as directed.
5. Encourage the individual to follow the doctor’s recommendations regarding healthy choices and wellness.
Other
1. Transport individuals as needed using a personal vehicle
2. Supervise general activities for the individual.
3. Handle emergencies based on organizational protocols.
4. Communicate with the supervisor and other staff using appropriate means.
5. Attend staff meetings and in-service training as scheduled and/or review / read the minutes of those meetings.
6. Participate in twenty-four hours of training as required by the regulations.
7. All other duties as assigned.
Job Requirements: Must be at least 18 years of age with a high school diploma or GED. At least two years of human services experience can be substituted for the high school diploma or GED. Additional education and/or experience in human services is preferred. Must also have a valid driver’s license, a vehicle with valid auto insurance, and valid car registration. Must submit to a criminal background check, an FBI fingerprint check, a Sex Offender Registry check, a driver’s record check, an initial physical, and a substance screening. This is a safety-sensitive position.
