**Job Description for Licensed Clinical Social Worker**
We are seeking a per diem / prn Licensed Clinical Social Worker to join our team at Jet Health in Denton, Texas.
Key Responsibilities
- Provide coordinated care in the home to patients of all age groups.
- Conduct ongoing clinical assessments to identify psychosocial, financial, environmental, and community resource needs.
- Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
The successful candidate will utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
Requirements
- A Master's or Doctoral Degree from an accredited school of Social Work.
- Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
- A minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
