The Intake Coordinators are first contact for all referral and facilities. Their role is to facilitate all functions at the point of a patient’s referral (registration, insurance verification, eligibility, etc.) In addition, the Intake Coordinator will be involved in staffing for clinical staff. The Intake Coordinator has direct contact with referring physicians, hospital liaisons and patient/families as it relates to Home Health benefits and services.
REQUIREMENTS:
Pay: $22.00 - $26.00 per hour
Expected hours: 24 - 30 per week

REQUIREMENTS:
- At least 1-year experience with Home Health Intake Coordinating (preferred).
- Experience with medical Intake or Scheduling is preferred
- Serves as the first line of customer service for internal and external customers.
- Receives and documents patient referrals to home health services, ensuring compliance with applicable coverage criteria.
- Gathers appropriate information to assist the clinical team in scheduling an evaluation visit.
- Coordinates the evaluation/admission process and may assign field staff to conduct initial assessment.
- Communicates pertinent patient information to the care center team.
- Experience with Clinical review of Home Health referrals.
- Maintain client data in a computerized scheduling system. Effectively use this system for tracking, information gathering, scheduling, and/or troubleshooting.
- Responsible for ensuring that all efforts are made to appropriately staff hours ordered for each patient and to keep accurate and timely communication with staff and families.
- Performs other related duties as assigned or requested.
- Schedule appointments and manage the intake process for new patients.
- Collect and verify patient information, including medical history and insurance details.
- Coordinate with healthcare providers to ensure timely and accurate scheduling of appointments.
- Maintain electronic medical records and ensure confidentiality of patient information.
- Assist with billing and insurance claims processing.
- Provide excellent customer service by addressing patient inquiries and concerns.
- Comprehensive knowledge of home health care.
- Demonstrates excellent observation and problem solving/critical thinking skills.
- Well-developed verbal and written communication skills; essential computer skills to work with EMR.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Self-directed and able to work with minimal supervision.
- Shows strong ability to organize and prioritize workload independently.
- Intake/Coordinating: 1 year (Preferred)
- Home Health/Care: 1 years (Preferred)
- Knowledge of Home Healthcare EMR (Preferred)
Pay: $22.00 - $26.00 per hour
