Responsibilities:
Pay: $37.00 - $40.00 per hour
Expected hours: 24 - 40 per week

Responsibilities:
- Conduct intake assessments and gather necessary information from patients
- Coordinate and schedule appointments for patients
- Assist with insurance verification and authorization processes
- Maintain accurate and up-to-date patient records in electronic health record systems such as Alora Plus, Well Sky, Axxess, and Homecare Homebase.
- Collaborate with healthcare providers to ensure appropriate care plans are implemented
- Provide support and guidance to patients and their families throughout the intake process
- Coordinate referrals to other healthcare professionals or community resources as needed
- Processes all daily patient referral and intake operations including all intake processes and policies.
- Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract providers.
- Communicates with multiple service providers to coordinate care.
- Follows up with patients in regards to Start of Care (SOC) & services being provided to measure satisfaction.
- Responsible for referral related phone calls.
- Calls physician to obtain orders when needed.
- Obtains verbal orders from physician when necessary.
- Follows up and reports delay stages, non-admits, and any other SOC issues.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Builds and monitors community and customer perceptions of the Company as a high quality provider of services.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the Company.
- Provides communication and support to Community Educators and or Community Liaisons referral sources and clients.
- Understands and practices agency policies and procedures.
- Stays current with CMS guidelines.
- Performs workflow and data reporting in accordance with CMS guidelines.
- Ensures compliance with all state and federal referral/intake regulatory requirements.
- Answers overflow phone calls.
- Maintains incoming faxes.
- Previous experience working in a healthcare setting, preferably in an acute care or behavioral health environment
- Familiarity with medical terminology, including ICD-10 code
- Experience working with children and/or individuals with behavioral health needs is highly desirable
- Knowledge of physical examination procedures, medication administration, and catheterization techniques
- Strong organizational skills and attention to detail to ensure accurate documentation and scheduling
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare providers
