Shift Schedule : Monday-Friday 9AM-5PM

General Responsibilities :
Under the supervision of the Supervisor of Residential Services, the Resident Service Coordinator is responsible for providing individual case management services and group programming for senior tenants.
Specific Duties and Responsibilities :
- Provides monthly in-home assessments of the social and emotional needs of the tenant.
- Assists formerly homeless tenants as they transition back to residing in the community.
- Develops and implements a written case plan to provide a full range of social services for the tenant and their family including :
- Provides individual and family counseling / guidance to resolve any issues the tenant is facing.
- Prepares specific information about the health, welfare, education, and recreational services available to serve the tenant including trainings for tenants regarding the obligations of tenancy.
- Assists tenants in developing informal support networks with other tenants, family, and friends, including facilitating opportunities for tenant engagement and participation in group meetings and recreational
activities.
