PURPOSE STATEMENT:

Preventive Services help keep children safe from abuse, neglect and maltreatment and help them remain at home and in their communities. We partner with at-risk families by assisting them to develop the skills to manage crisis and maintain safety and stability by offering a range of clinical and concrete services to help them thrive. Our culturally competent staff addresses the individual needs of the child, each family member residing with the child, and their social economic environment. Families are seen primarily in their homes and communities.

POSITION OVERVIEW:

The Case Planner works with the entire family in evidenced based or non-evidenced based model programs. The Case Planner makes safety-focused assessments of the child and family, and provides counseling services, advocacy and refers for concrete services to help stabilize the environment, improve family well-being, and to reduce the future need for child welfare involvement.

KEY ESSENTIAL FUNCTIONS:

  • Provide Child Welfare Preventive Services exclusively or primarily within the client’s home and community.
  • Assess and address any safety and risk concerns related to children’s health, safety and well-being.
  • Evaluate living conditions of clients through home base contacts.
  • Provide supportive case work counseling to children and family members.
  • Identify and provide access to specific concrete services and community resources, when necessary or requested
  • Collaborate with external resources in the family’s ecology e.g. school, medical provider, mental health providers, CPS worker, probation, extended family and community members, etc.
  • May conduct specialized groups for children, adolescents, parents, etc.
  • Attend conferences at ACS offices and other community-based meetings.
  • Complete weekly progress notes in CONNECTIONS and enter data into PROMIS
  • Complete and update timely Family Assessment Service Plan Reports (FASP).
  • Attend ACS and JB trainings as required.
  • Carry caseload size as identified by program service model.
  • Assess and refer children and families to supportive and concrete services, i.e. mental health, substance abuse, housing, financial assistance, legal, etc.
  • Participates in internal and external continuous quality improvement projects.
  • Monitor children regarding their medical needs, such as compliance with periodic physicals and immunizations
  • Participate in Family Team Conferences (FTCs).
  • Performs outreach to communities and establish linkages with other groups, individuals or organizations which may be helpful to clients.
  • Complete agency internal Incident Reports in Alltrac system.
  • Assess level of interactions of all family members in household including other adults frequenting the home.
  • Make report to State Central Registry when required.
  • Identify and monitor high risk families on caseload who are at high risk of abuse, neglect and further system involvement.
  • Maintain the case records consistent with JBFCS and ACS standards.
  • Work a hybrid schedule based on program needs
  • Additional tasks as assigned

CORE COMPETENCIES:

  • Basic social work knowledge and understanding of stages of change
  • Ability to provide in-home child welfare services
  • Ability to establish rapport and therapeutic relationship with families
  • Understanding of the use of family therapy principals to provide services (preferred)
  • Understanding of early childhood development and related theories and models of practice (preferred)
  • Strong Assessment skills to evaluate dynamics and interactions within the family (preferred)

EDUCATION / TRAINING REQUIRED:

  • BA or Masters level degree in Psychology, Social Work, Counseling or a related subject

COMPUTER SKILLS REQUIRED:

  • Knowledge of Microsoft office required
  • Knowledge and experience with the Connections system preferred

VISUAL AND MANUAL DEXTERITY:

  • Limited applications of manual dexterity and hand-eye coordination.

WORK ENVIRONMENT / PHYSICAL EFFORT

  • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds
  • Risks/hazards associated with the position are those which may be encountered travelling around New York City and interfacing with clients experiencing an acute mental health crisis or serious emotional disturbance
  • Travel in the community and home-based work.
  • Position is currently hybrid but can become 100% in person based on program need.
