High Needs Case Manager :   JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families / caregivers of the youth receiving services.

Responsibilities

  • Provides case management services to children enrolled in the behavioral-health system.
  • Provides weekly contact with assigned clients, which includes home / community visits, attending medication checks, etc.
  • Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
  • Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation / Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Coordinates and facilitates Child & Family Team (CFT) Meetings.
  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
