JFSA Caring at Home has an immediate opening for a PRN Occupational Therapist to provide comprehensive, professional, and compassionate therapy services to clients in their homes. The Occupational Therapist assesses and treats clients according to the individualized plan of care; monitors and documents the client's condition, progress, and outcomes; and provides client and caregiver. Services are provided under physician order and in accordance with home health agency regulations, federal and state laws, and agency policies.
Duties & Responsibilities:
- Provides OT assessments, diagnostic tests and skilled treatments, and ongoing evaluations of clients who are receiving services under a medically approved Plan of Care.
- Participates in the development of the Plan of Care.
- Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
- Prepares a report of each client’s diagnostic evaluation and makes recommendations.
- Prepares clinical and progress notes.
- Instructs clients and their families in Activities of Daily Living.
- Maintains confidentiality in all aspects of the job.
- Attends care conferences as deemed necessary.
- Participates in in-service programs as requested.
- Performs other duties as assigned.
