We are seeking a CRDA Program Case Manager who will be committed to developing partnerships and conduct targeted outreach to connect with key populations who might otherwise miss the supports they need to thrive. The Case Manager will serve all ages and working specifically with the unmet needs of Atlantic city residents. As the case manager, you will meet with individuals and/or families to offer linkage to resources to increase the likelihood of success within the community. You will need to be a good listener and good communicator. In response to individual client or family needs, you will work to support wellbeing in many areas, which may include mental health, medical, housing, food security, education, vocation, and legal challenges. You will also be conducting outreach to Atlantic City casino employees to increase awareness and access to services for themselves and their families. Duties/Responsibilities:
- Participate in community meetings to develop partnerships and communication with professionals who serve Atlantic City residents.
- Complete intakes with new referrals to assess needs
- Provide case management services that include linkage and referrals to JFS programs and other appropriate agencies for community based services
- Conduct outreach to Atlantic City businesses, organizations, services, and residences in order to develop connections and facilitate referrals to case management services as well as other JFS and community resources
- Maintain required agency documentation, including an individualized goal plan
- Participate on the agency’s cultural diversity committee and/or anti-racism committee
Education/Experience:
- A Bachelor’s degree in Social Work or a related field. Two years of experience in the mental health field preferred.
- Flexibility as far as schedule is concerned (Day, Evening, Full- and Part-Time, Per Diem).
- Bilingual in English and Spanish preferred
