Case Manager - (Homeless Services - PATH Program)

Key Responsibilities:
As a Case Manager for Homeless Services in our PATH program, you will play a vital role in assisting homelessness facing various challenges. Your responsibilities will include:
- Assessing the needs of clients and developing individualized care plans.
- Connect clients with essential community resources, including housing programs, healthcare services, employment opportunities, substance abuse treatment, and mental health support.
- Ensure clients have access to the required services and that their rights are respected.
- Provide guidance and support to empower clients to regain their independence and work towards stable housing and self-sufficiency.
Qualifications:
- Compassion, empathy, and commitment to serving homelessness.
- Strong communication and interpersonal skills.
- Excellent organizational and problem-solving abilities.
- The capacity to work both independently and as part of a team.
- Dedication to upholding high ethical and professional standards.
Position Requirements:
- Education: Bachelors in Social Work, Psychology, Human Services or other related fields
- Experience: 0 or more years of experience in a mental health, social work, or related setting
- · Valid Driver’s License required
