The ICMS Case Manager/Hospital Liaison, coordinates and delivers client-centered services, including developing service plans, facilitating access to resources, providing outreach and support for clients diagnosed with severe mental illness (SMI). Working as part of an interdisciplinary team, the ideal candidate for this position helps clients set goals, build skills, and integrate into the community while ensuring compliance with documentation and program standards. The hospital liaison works interactively with the ICMS team, the consumer and the Psychiatric units to ensure successful community integration.
Skills and Responsibilities:
- The delivery and coordination of services to consumers includes:
- Individual, group and family intervention
- Outreach case management and support
- Referral and advocacy for linkage to essential services based on consumer choice
- Connection to social, spiritual and other community resources as expressed by the consumer
- Ensuring documentation and productivity per agency standards
- Maintain weekly contact with psychiatric hospital social work departments to ensure appropriate referrals and to reinforce the continuum of care for existing consumers.
Provide assessments and program descriptions to potential ICMS consumers at psychiatric hospitals
- Act as the liaison for the psychiatric units and the ICMS team.
- Visit consumers as needed on the psychiatric unit and support with discharge planning and community integration success.
- Visit psychiatric units in person to interact with current and potential consumers to build rapport, provide information about the program and assess the consumer’s needs and appropriateness for the program. Communicate with the psychiatric social workers to help create appropriate discharge planning.
Qualifications
Bachelor’s Degree in Social Work or related Human Service (Psychology, Behavioral Health, Counseling, Psychiatric Rehabilitation). One year of residential MH setting is required .
