Job Summary
The AHA Outreach Case Manager plays a crucial role in connecting individuals experiencing homelessness or at imminent risk of homelessness with essential services. Working under the Atlantic Homeless Alliance (AHA), the Case Manager provides client-centered, efficient, and equitable care coordination. This role involves outreach efforts in both community and agency settings to conduct HMIS intakes, complete assessments, and develop individualized action plans. The Outreach Case Manager will also facilitate connections to social service resources, shelters, and assisting clients in navigating benefits and entitlements as part of comprehensive support.
Duties/Responsibilities
Outreach and Client Engagement
- Conducts outreach to individuals at risk of homelessness or currently homeless, ensuring they are connected to intake services through AHA or referred to other suitable community resources.
- Completes comprehensive HMIS (Homeless Management Information System) intakes and assessments both on-site at AHA and off-site at shelters, community partner agencies, and other relevant locations.
- Develops action plans tailored to individual client needs, supporting them in achieving stability and securing housing.
Service Coordination and Referrals
- Coordinates incoming service requests and triages them for appropriate distribution to AHA case workers and programs.
- Facilitates referrals to community resources, including benefits, entitlements, housing programs, and other social services.
- Builds and maintains positive relationships with community agencies to streamline referral processes and enhance client service options.
Documentation and Compliance
- Prepares timely and accurate documentation of all client interactions and services rendered, in compliance with agency standards and billing requirements.
- Ensures all client records are maintained accurately within agency information systems, meeting compliance standards and supporting program billing.
Collaboration and Professional Development
- Actively participates in team meetings, supervision sessions, and in-service training workshops to stay informed of program policies, procedures, and best practices.
- Demonstrates a high level of professional integrity and adherence to the NASW Code of Ethics.
- Meets or exceeds productivity and engagement standards as outlined by the AHA program and agency.
Administrative Support
- Provides general support for the AHA program by managing phone calls, handling walk-in requests, and coordinating intake rotations.
- Collaborates with program supervisors to ensure client referrals are efficiently distributed across programs and resources
Requirements

- Bachelors in Social Work, Psychology, Behavioral Health or other related human service field.
- Bilingual preferred (Spanish/English)
- β Monday-Friday 9am-5pm at the Atlantic Homeless Alliance, located in the Atlantic County Board of Social Service Building
