As part of a multi-disciplinary team, the Case Manager  advocates for, initiates and implements all aspects of a client's case plan concerning access to required resources, services, and opportunities according to established agency standards.

QUALIFICATIONS:            

  • Bachelor’s degree in social work or related field or equivalent experience. 
  • Minimum of 2-year experience working with victims of domestic violence in a shelter setting
  • 40-hour domestic violence training required.  
  • Bi-lingual (English/Spanish) required.
  • Demonstrate ability to develop, coordinate, document effectively and engage clients to create and implement their case plans.
  • Ability to communicate effectively, perform crisis intervention, 
  • Familiarity with social services, community agencies, transitional housing, and other resources, and the ability to effectively link clients with needed and available opportunities.  
  • Ability to perform community presentations, perform outreach, and build partnerships within the community.
  • Be culturally sensitive and able to work with a multi-cultural client/customer population; ability work in a non-judgmental manner and foster positive outcomes; and ability to work as part of a team and maintain professional boundaries.  
  • Intermediate computer skills including familiarity with Microsoft Office and databases used for grant funding..
  • Valid driver’s license, current auto insurance and registration    
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

                                    

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Manage case plans of shelter program clients, including:
    1. keeping statistical records
    2. assisting clients with establishing their goals and personal budget
    3. monitoring progress toward goals and assisting with activities related to the case plan
  • Develop and assist in implementation of case plans in collaboration  with shelter residents 
  • Answer crisis line in shelter and assist callers with service needs.
  • Enter information into grant databases as required.
  • Responsible for provision of shelter coverage and assist with all aspects of daily shelter service delivery.
  • Act as liaison to community and attend grant related meetings or other meetings as assigned.
  • Other duties as assigned.
