Role & Responsibilities:
- Provides both short- and long-term counseling using evidence-based practices and interventions.
- Conducts client assessments to evaluate needs and functionality, monitors progress through ongoing reassessment, and makes referrals to additional service providers (e.g., healthcare, housing, financial assistance).
- Collaborates with clients and onsite psychiatrists to develop and update personalized treatment plans.
- Connects individuals and families to appropriate community resources, serving as a liaison and advocate with community organizations to support client well-being.
- Ensures documentation compliance by preparing and completing all required progress notes, safety plans, records, and reports promptly in an electronic database.
- Adheres to standards set by external accrediting and licensing bodies, attending all assigned staff and committee meetings.
- Participates in agency in-service programs, workshops, supervision, and team meetings.
- Offers evening availability for clients unable to attend daytime appointments, with two evening sessions per week.
- Protects client confidentiality and maintains trust by securing sensitive information in accordance with agency protocols.
