Position: Geriatric Care Manager

Overview of Role:
The Geriatric Care Manager will provide assessment, support, and resource management for older adults in the community, including home, phone and office visits to older adult clients residing in the community. Care Manager will assist with access to services guided by comprehensive needs assessment and work as a part of a team that interacts with other programs and departments in the provision of care.

Role & Responsibilities:
- Assess client functioning to determine service needs, provide ongoing monitoring, support and resources via intakes, monthly check-ins and ongoing visits
- Maintain caseload of older adults and offer support, resource management, and care coordination
- Interact with agency departments as r ole indicates
- Update care plans and maintain timely records and documentation
- Refer individuals and families to appropriate community services and act as liaison and/or advocate with community organizations to enhance client functioning
- Maintain client documentation standards by preparing and completing, in a timely manner, required progress notes, records, and reports for assigned clients, in an electronic database
- Comply with standards and requirements of all external accrediting/licensing bodies by attending staff meetings and committee meetings as assigned
- Attend internal/external programs, workshops, supervision, and team meetings
- Maintain client confidence and protect operations by keeping information confidential
